Initial Comments:
An onsite follow-up survey conducted on June 28, 2023, found that Dialysis Care Center Lebanon, LLC had corrected the following deficiencies cited under 42 CFR, Part 494, Subparts A, B, C, and D, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services. The deficiencies were cited as a result of a Medicare recertification survey that was completed on May 17, 2023.
As a result of the survey, the condition level citation has been lifted.
